I hereby grant permission for my son/daughter to join the class.

You must be at least 16 years old to attend this class. You do not need a permit to attend this class. If you are under 18 years old, one parent must attend class with you on Sunday from 12:30-2:30pm. The total fee for the class is $150.00. A non-refundable deposit of $50 will be required to hold your spot. Paypal charges an extra $2 fee. $100 cash will be due on the first day of class. You will receive your certificate of completion (CS-1) on the last day of class.
